CPT 95851: range of motion measurements

2026-10-18 · 8 min read

95851 is the code for a formal range-of-motion measurement with a written report. It is real and separately priced, and on most of the days a therapist or chiropractor would think to bill it, Medicare's edit tables have already folded it into something else.

Medicare's unit ceiling for 95851 (95852: 1)

untimed

one unit per extremity or spinal section, however long it took

95851 covers measuring a patient’s range of motion and writing it up as a report, counted per extremity (the hand is excluded) or per section of the spine. 95852 is its sibling for the hand, whether or not you compare with the other side. Both sit in the medicine section of CPT beside the neuromuscular tests, not in the 97000 physical medicine family, which is the first clue to how they behave on a claim.

The manual muscle-testing codes that used to travel with them, 95831–95834, left CMS’s therapy code list on 12/31/2019. Range of motion survived that cleanup, and the edits below explain why it still rarely reaches a claim beside an evaluation code.

Units: per region, not per minute

Nothing in the 95851 definition mentions time, so Medicare treats it as untimed: one unit for each region measured, and the 8-minute rule never touches it. A full lower-quarter screen of both legs is two units whether it took six minutes or twenty.

arm · 1 unitleg · 1 unitspinal section· 1 unit eachhand · 95852
Medicare caps 95851 at 3 units and 95852 at 1 per day (MUE).

The practitioner MUE caps 95851 at 3 units on one date of service and 95852 at 1. Four limbs plus a spine is more than three, so a “measure everything” day loses units at the claim edit before anyone reads the note.

The edits that decide it

In the Medicare NCCI practitioner file, 95851 is the Column Two code against every PT and OT evaluation and re-evaluation, every office E/M visit, spinal chiropractic manipulation, therapeutic activities, group therapy and the physical performance test. All of those carry modifier indicator 0: 59, XU or any other modifier will not unlock them. Only manual therapy carries a 1.

Can 95851 go on the same day as…

Pick the other code on the visit. Verdicts read straight from the Medicare practitioner PTP file; 95852 follows the same edits except where the table above says otherwise.

95851 + 97162

Not separately payable the same day

97162 (pt evaluation, any complexity) is the Column One code; 95851 is Column Two with modifier indicator 0. No modifier opens this edit — the measurement is paid inside 97162.

The eval-day goniometry claim

Billing 97162 plus two units of 95851 on day one is the commonest way to meet this edit. The range you measured at the evaluation is part of the evaluation. It belongs in the objective section, not on its own claim line.

When it is billable

With 97110, 97112, 97116 the file has no pair at all, so a treatment visit built on therapeutic exercise, neuromuscular re-education or gait training will not trip an edit, and the modality codes raise no pair with it either. That is not the same as saying 95851 belongs there. CMS’s NCCI manual describes the 95851–95941 range as significant, separately identifiable diagnostic services that need a formal report in the record. The quick goniometer check that told you whether to progress today’s exercise is part of the exercise.

The defensible case is a planned, formal measurement on a treatment day that is not an evaluation or re-evaluation — a post-operative knee measured bilaterally for a progress report, with a written result the surgeon will read.

Therapy status: sometimes therapy

CMS lists 95851 and 95852 as disposition 7, “sometimes therapy”. Always therapy when a therapist furnishes it (GP/GO/GN modifier, plan of care); not therapy when a physician or NPP bills it outside a therapy plan of care. In practice: billed by a PT it takes GP and counts toward the KX threshold like any other therapy line.

What the report needs

A report that holds up names the joint and motion, active or passive, the position, the instrument and landmark, both sides, and the previous value taken the same way. The goniometer technique guide covers why the position and landmark matter more than the number.

Knee extension, AROM−6°0°0°−11°

Supine, standard 12-inch goniometer, axis at the lateral femoral epicondyle, same clinician as the prior measurement. Example values; reference from the site’s ROM table.

Compare against the patient’s other side first and the population table second — the range of motion chart and the knee norms carry the reference values and how far published sources disagree.

What it pays

Both codes are status A on the 2026 fee schedule: active and paid separately when no edit stands in the way, with a XXX global period. The relative values are small.

One quirk in its favour: 95851 carries multiple-procedure indicator 0, so it does not take the practice-expense reduction that indicator 5 therapy codes such as 97110 take when several are billed together.

A measurement worth about $26 is never worth a claim line that reads like unbundling.

For chiropractors

The pair with spinal manipulation is one of the oldest in the file: 98940–98942 over 95851, in effect since 1997-10-01, modifier indicator 0. The office E/M pairs followed on 2020-10-01. For a chiropractor billing Medicare, that leaves no same-day route — a re-examination on a manipulation day already carries its own 25-modifier question, and range of motion does not ride along with it.

Record the measurements in the exam findings, where they support the treatment plan and the AT modifier, and bill the visit on the manipulation and the services that genuinely stand apart from it.

Commercial payers write their own rules

NCCI binds Medicare claims. A commercial or auto payer may pay 95851 where Medicare will not, or bundle it more tightly. The payer’s own policy and your contract decide, so read them before building a template around the code.

Red flag — refer out

Range that keeps falling across measurements despite treatment, or a sudden loss with pain out of proportion to the mechanism, is a clinical finding to act on, not a number to bill.
