Billing

Billing & Coding

4 tools · 10 guides · 14 pages

CPT units, timed-code arithmetic, chiropractic manipulation codes and the superbill a cash patient actually needs.

Two arithmetics live on this shelf and they never mix. Physical therapy bills most of its procedures in timed 15-minute units, so the minutes in the note decide the claim; start with the units calculator and the single-code guides such as 97110 and 97116. Chiropractic manipulation is billed per visit by the number of spinal regions treated, which is a counting problem rather than a clock problem; the chiropractic billing guide covers the region count and the AT modifier.

Everything else here sits between those two: which codes are untimed, which modalities Medicare bundles (see the modality code guide), when a range-of-motion measurement is separately billable, and what the KX modifier attests once a patient passes the annual therapy threshold. Descriptions of CPT codes are our own paraphrase; the official descriptors belong to the AMA.
