Chiropractic SOAP note generator

A chiropractic daily note has one extra job over its physical therapy cousin: it has to prove the region count behind the CMT code and answer, in words, whether the care is still active.

Name the regions, not the segments

“Adjusted C2, C5, T4” is three segments in two regions, which is 98940. Writing the segments is good clinical practice and, on its own, useless for the code — a reviewer counts regions. The lookup at the chiropractic code table lists what falls inside each of the five, including the sacral / pelvic split that catches people out.

Answer the AT question in the assessment

Medicare covers chiropractic manipulation only for active treatment of subluxation. Without AT the claim reads as maintenance and is denied; with AT on a record that reads like maintenance, it is a compliance problem rather than a coding one.

A pre and a post number

The strongest chiropractic notes carry one measurement twice: before the adjustment and after. It takes ten seconds with a goniometer or an inclinometer, it makes the assessment write itself, and it is the difference between a note that describes a treatment and one that demonstrates an effect. The maintenance conversation

Maintenance care is a legitimate service that Medicare does not cover. The failure mode is not providing it — it is providing it while the notes still read as active treatment. Say which one you are doing, tell the patient which one they are paying for, and let the coding follow.

Red flag — refer out

Manipulation over a suspected fracture, an unstable segment, progressive neurological deficit or an unexplained systemic picture is not a documentation problem. Stop and refer.
